Exercise Physiology Information
We deliver person centred, safe, effective and professional care. Our experienced, friendly team have developed the following policies to make this possible:
You will be individually assessed prior to undertaking any exercise. Your Exercise Physiologist will collaborate with your other Health Professionals to coordinate and deliver the best possible care. Every client will be educated and supervised to ensure safe and effective outcomes. Your exercise routine will be tailored to your individual needs and you will be expected to complete exercises in your own time to complement the supervised appointments. You will be given clear and easy to follow guidelines for your home exercises. Exercise routines will be detailed in writing and be clear and easy to understand. You are encouraged to ask questions and discuss your needs to enhance your outcomes.
Fees for our services are due at the time of service.Hicaps and EFTPOS facilities are available at the front desk for automatic claiming through your private health fund. WorkCover, TAC and DVA patient accounts will be sent directly to the appropriate body, but must be paid for until confirmation of the claim.
Appointment Scheduling and Cancellations
Please make every effort to keep your scheduled appointments as suggested by your Exercise Physiologist. Scheduling your appointments in advance will help you secure a convenient time. All cancellations must be received by the clinic at least 48 hours prior to the scheduled appointment, otherwise the full fee will be charged.
Please call the clinic directly on (03) 9481 2955 to cancel or re-schedule, or Mike on 0438813022. Please note cancellations for Monday appointments not received by 5pm on the Friday before will be considered as a late cancellation and the full fee will be charged.
Mobile Phones and Valuables
We prefer you to be attentive, so unless you are expecting an urgent call, please put your phone to silent during your appointment. Valuable items may be stored in the clinic during your appointment, but the security of your valuables is your responsibility.
Please bring a towel and a water bottle to your appointment. Wear clothing that you will feel comfortable in. Changing room and toilet facilities are provided. Please note there are no showers on site. A water dispenser is available for filling water bottles. Access to The Exercise Clinic equipment and facilities is by appointment only.
Clifton Hill Physiotherapy respects the privacy of each individual. We are committed to the protection of personal information in accordance with privacy laws. The personal information collected by our clinic is collected to ensure optimal care and for administrative purposes. The physiotherapists use shared files so personal information will be available to any therapsit that the client consults.
Clifton Hill Physiotherapy will disclose health or personal information to an authorised 3rd party only where the individual has provided written authority. Clifton Hill Physiotherapy stores personal information in paper and electronic form. All reasonable steps are taken to protect this this information in secure storage. Personal and health information is retained as per privacy laws and will be disposed of in a secure manner.
The Clifton Hill Physiotherapy website does not record personal information other than the information you provide when you contact us. You can access the website anonymously. Email addresses and other contact details you provide will not be added to mailing lists without your consent. We will not disclose your personal information to a third party without your consent with the exception that we are required or authorised to do so by law or other regulation.
